It does happen sometimes. Though most animals don't really have a reason to attack humans and try to avoid them. Most of them have clear well defined boundaries on where the people are allowed to go and guides respect them.
The guides are also trained to read the mood of the animal, should they come close and know how to handle such events before they become aggressive. Like by spreading silly superstitions like the ones that as long you remain seated, animals cant tell there are people in the car. So its very rare to see attacks, provided the guests dont do anything stupid. 

But a quick search on youtube gives plenty of examples. Though in most cases the animals are clearly gotten to used to the presence of humans or curious subadults. Reply
